BACKGROUND: There is not a prevailing consensus on appropriate antibiotic choice, route, and duration in the treatment of bacterial pleural empyema after appropriate source control. Professional society guidelines note the lack of comparative trials with which to guide recommendations. We assessed clinical outcomes in the treatment of known and suspected empyema based upon three aspects of antibiotic use: (1) total duration, (2) duration of intravenous (IV) antibiotics, and (3) duration of anti-anaerobic antibiotics. METHODS: We performed a hypothesis-generating retrospective chart review analysis of 355 adult inpatients who had pleural drainage, via either chest tube or surgical intervention, for known or suspected empyema. The primary outcome variable was clinician assessment of resolution or lack thereof. The secondary outcomes were death within 90 days, hospital readmission within 30 days for empyema, and all-cause hospital readmission within 30 days. Mann-Whitney U test was used to compare outcomes with regard to these variables. RESULTS: None of the independent variables was significantly associated with a difference in clinical resolution rate despite trends for total antibiotic duration and anti-anaerobic antibiotic duration. None of the independent variables was associated with mortality. Longer total antibiotic duration was associated with lower readmission rate for empyema (median 17 [interquartile range 11-28] antibiotic days in non-readmission group vs. 13 [6-15] days in readmission group), with a non-significant trend for all-cause readmission rate (17 [11-28] days vs. 14 [9-21] days). IV antibiotic duration was not associated with a difference in any of the defined outcomes. Longer duration of anti-anaerobic antibiotics was associated with both lower all-cause readmission (8.5 [0-17] vs. 2 [0-11]) and lower readmission rate for empyema (8 [0-17] vs. 2 [0-3]). CONCLUSION: Our data support the premise that routine use of anti-anaerobic antibiotics is indicated in the treatment of pleural empyema. However, our study casts doubt on the benefits of extended IV rather than oral antibiotics in the treatment of empyema. This represents a target for future investigation that could potentially limit complications associated with the excessive use of IV antibiotics.

Although organ hypofunction and immunosuppression are life-threatening features of severe sepsis, the hypofunctioning organs and immune cells usually regain normal functionality if patients survive. Because tissue interstitial fluid can become acidic during the septic response, we tested the hypothesis that low extracellular pH (pHe) can induce reversible metabolic and functional changes in peritoneal macrophages from C57BL/6J mice. When compared with macrophages cultured at normal pHe, macrophages living in an acidic medium used less glucose and exogenous fatty acid to produce ATP. Lactate, glutamine, and de novo-synthesized fatty acids supported ATP production by mitochondria that gained greater mass, maximal oxygen consumption rate, and spare respiratory capacity. The cells transitioned to an M2-like state, with altered immune responses to LPS and slightly decreased phagocytic ability, yet they regained basal energy production, normal mitochondrial function, and proinflammatory responsiveness when neutral pHe was restored. Low pHe induces changes that support macrophage survival while rendering the cells less proinflammatory (more "tolerant") and less able to phagocytose bacteria. Macrophage responses to low interstitial pH may contribute to the reversible organ hypofunction and immunoparalysis noted in many patients with sepsis.

Various lepidopteran insects are responsible for major crop losses worldwide. Although crop plant varieties developed to express Bacillus thuringiensis (Bt) proteins are effective at controlling damage from key lepidopteran pests, some insect populations have evolved to be insensitive to certain Bt proteins. Here, we report the discovery of a family of homologous proteins, two of which we have designated IPD083Aa and IPD083Cb, which are from Adiantum spp. Both proteins share no known peptide domains, sequence motifs, or signatures with other proteins. Transgenic soybean or corn plants expressing either IPD083Aa or IPD083Cb, respectively, show protection from feeding damage by several key pests under field conditions. The results from comparative studies with major Bt proteins currently deployed in transgenic crops indicate that the IPD083 proteins function by binding to different target sites. These results indicate that IPD083Aa and IPD083Cb can serve as alternatives to traditional Bt-based insect control traits with potential to counter insect resistance to Bt proteins.

The creation of new cluster building blocks, as well as new ligand coordination modes, are among the most effective ways to develop new framework materials. Yet, large and chiral clusters are both difficult to create and relatively few. Here, by studying the competing coordination of different azolates against carboxylate and combined carboxylate/phenolate, it is shown that the impact of azolates in the MOF-74 synthesis system differs dramatically, leading to the synthesis of MOF-74, UTSA-74, and CPM-72 for 2-methylimidazole, 1,2,4-triazole, and 1,2,3-triazole, respectively. The new CPM-72 contains a novel chiral Zn12 triazolate cluster, which features a trigonal-prismatic Zn6 core inside an octahedral Zn6 shell. In contrast with MOF-74 with fully deprotonated and symmetrically bonded 2,5-dihydroxyterephthalic acid (H4 DOBDC), H4 DOBDC adopts an unusual nonsymmetric bonding mode in CPM-72 (carboxylate only at one end and carboxylate/phenolate at the other), resulting in a highly porous and intrinsically chiral 3D framework. The nonsymmetric bonding mode by H4 DOBDC, apparently dictated by the chiral Zn12 cluster, can be replicated with 2-hydroxyterephthalic acid (H3 OBDC), leading to the synthesis of porous isoreticular CPM-73.

The preparation of a variety of sulfonated carbons and their use in the esterification of oleic acid is reported. All sulfonated materials show some loss in activity associated with the leaching of active sites. Exhaustive leaching shows that a finite amount of activity is lost from the carbons in the form of colloids. Fully leached catalysts show no loss in activity upon recycling. The best catalysts; 1, 3, and 6; show initial TOFs of 0.07 s(-1), 0.05 s(-1), and 0.14 s(-1), respectively. These compare favorably with literature values. Significantly, the leachate solutions obtained from catalysts 1, 3, and 6, also show excellent esterification activity. The results of TEM and catalyst poisoning experiments on the leachate solutions associate the catalytic activity of these solutions with carbon colloids. This mechanism for leaching active sites from sulfonated carbons is previously unrecognized.
